When I last used it a year or so ago, I couldn't get the conversion to work and had to convert to M4A externally and then let CaV just build the M4B from the M4A files. It's interesting because you can make mp3's into a book without chapters almost instantly (just 'zip' them up with no compression, then rename the zip file to mp3 and use any mp3 tool to add tags), but it's almost impossible to add chapters to such a book.
